Gina Cappelli started her career as a partner in Cappelli Associates, a family owned real estate development company, with more than 20 years experience in property management, real estate acquisitions, corporate finance, leasing and personnel management. In her position, she provided management oversight for more than 1.2 million square feet of corporate office space.

In June 2000, Ms. Cappelli ventured away from the family business and Founded Formé Rehabilitation, Inc., a NYS licensed Article 28 Diagnostic & Treatment Center and a NYS Recognized Woman Owned Business, with locations in Scarsdale and Eastchester, providing physical and occupational therapies to the local communities.
It has always been the vision of Ms. Cappelli to provide patients with high-quality and compassionate medical care, so in 2014 Ms. Cappelli closed the physical therapy locations and constructed a new 11,000sf state-of the-art medical facility in White Plains, New York, today known as Formé Medical Center. The facility now operates as a full service D&TC, which provides Urgent Care, Primary Care, Gynecology, Cardiology, Neurology, Podiatry, radiology and diagnostics services.
In 2015, Ms. Cappelli recruited Maria Trusa as the new CEO of Formé Medical Center. Together the two were determined to create a place for the underserved community to get access to affordable, dignified, and high quality medical care. In March 2016 Formé introduced an extremely unique program known as the “Formé Medical Membership” to the Medical Center. With this program members get unlimited urgent care and primary care services, as well as major discounts for laboratory services, prescription drugs, specialist, dentistry, surgical centers and more. Anyone without medical insurance can become a member and everyone qualifies for this program regardless of residency, financial or immigration status.

In 2020, when the world came to a complete stop with COVID-19 pandemic, Ms. Cappelli and Ms. Trusa created one of the first walk-up COVID testing center for the underserved community in Westchester County. Understanding that this population did not have access to testing due to their lack of transportation and legal documentation, Formé and it’s doctors and staff went the extra mile to ensure that testing was done, vitals were checked and care was given for this community so desperately in need of these services.
Today, Ms. Cappelli and Ms. Trusa, along with their dedicated and compassionate team, continue to test and provide high-quality healthcare to the community for all their healthcare needs!
